Following discharge from a hospital or clinic, some individuals have specific care and medical needs that are too complex for family and friends to manage alone. Transitional Care at the Walker Methodist Health Center offers round-the-clock skilled nursing care in the East Harriet neighborhood of Minneapolis.
Personalized care and focused rehabilitation services along with innovative Health Center Healing Therapies allow for a safe and smooth transition back to home and daily lifestyle.
Our inter-disciplinary team includes nursing, social services, therapy, and a dietitian who collaborates with our on-site medical providers to understand your care needs and therapy goals.
Traditional Transitional Care
We believe in fostering a strong connection with you and providing a unique, personalized care plan. You will be assigned to a therapy team composed of our highly trained Health Center professionals — we do not employ outside therapy teams.
Physical, occupational, and speech therapies are available six days a week. Therapy is available six days a week. In addition, all of our Transitional Care staff are certified in the TULIPS training program developed by the Struthers Parkinson’s Center.
Delirium/Memory Care Transitional Care
Delirium care is a unique feature of the Health Center and our model was the first of its kind in the Twin Cities. Offering all-private rooms, the neighborhood is designed for people with delirium, dementia or other types of short-term cognitive impairment.
